Forms for ScholarWorks
How Do I Complete the Forms?
All the forms are in Microsoft Word or Adobe PDF format. Though the Word files can be saved to your computer, the PDF forms must be filled out online. Please enter all necessary information using the check boxes, drop down menus, and blank fields. If you need to start over, click the Clear Form button to erase all the fields. When you have finished filling out the form, print two hard copies: one for your records and one to submit.
NOTE: As soon as you close a PDF form, all the entered information is cleared, so be sure to completely finish and print the form before closing.
The last section of most forms contains signature lines that are not interactive. Therefore, most printed forms will need to be submitted to an advisor, program coordinator, or dean for review and approval. Please note that for forms with signature approval lines, we need the ORIGINAL form with ORIGINAL signatures, not a photocopy, email attachment, or scan. These documents become part of your permanent Boise State file.

When Are the Forms Due?
Deadlines are listed in the Academic Calendar, which spans several semesters and lists deadlines for every stage of the graduate process.
